I figured it out.
Here is the working HAl and INI files just in case this can help someone else.
Thanks for the help everyone.
Now for tuning more IO and Toolchanger setup.

Very strange, that you did not get the A axis Buttons on the fly.
Let me check that, you may have stept over a bug.

Could you please just open a Sim config with an A axis and see if the buttons are there?

That's what I get after pulling recent LinuxCNC 2.8

@Chris: I did not correct that bug, have you done so? If so, thanks! Could you please in future changes in gmoccapy also change the release number? That way it is easier for me to support different releases.

Yes, I change the release number after every fix or adding stuff.
you will find a relase notes file in the source directory of gmoccapy, that way I can see easy if I already fixed stuff in a later release.
